Here at Webster Park Rehabilitation & Healthcare Center we are proud to be an American Heart Association Heart Failure Certified Skilled Nursing Center and eager to provide qualitative cardiac rehabilitation for those entrusted within our care.
Benefits of being an AHA Heart Failure Certified Center:
Certification: Less than 1% of skilled nursing facilities (SNFs) nationally have achieved this American Heart Association certification. ^1
Confidence in our care and outcomes: The hospital and patients can have confidence that we have been highly vetted by the AHA expert review process to meet their heart failure care standards. ^1
Improved care coordination: Certification improves health care coordination and communication between the hospital and our SNF, thereby enhancing the continuum of care for the patient for improved outcomes. ^1
Reduced readmissions: Heart failure patient care in our SNF impacts readmission rates, positively or negatively. AHA Heart Failure certification helps reduce unnecessary readmissions to the hospital. ^1
Assurance for hospitals: The certification program gives referring hospitals assurance that their patients will have strong continuity of heart failure care upon discharge to our facility. ^1
The American Heart Association is a relentless force for a world of longer, healthier lives for all.
